PHILIP A. GILLY, M.D. (Authorization No. 180754-4B)

Please be advised that, by Consent Order (BPMC No. 07-185) of the New York State Department of Health, State Board for Professional Medical Conduct, effective August 31, 2007, Dr. Philip A. Gilly's license to practice medicine in the State of New York was suspended for three years, stayed with probation for three years. Dr. Gilly admitted guilt to the charge of negligence on more than one occasion.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Gilly from the list of authorized physicians, effective September 28, 2007. Dr. Gilly's current address is 1301 River Street, Suite 101, Valatie, New York 12184.

Dr. Gilly is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Gilly for treatment or examinations rendered prior to September 28, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Gilly regarding treatment or examinations he rendered prior to September 28, 2007, should not be denied due to the suspension of his license or his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through September 27, 2007, may be provided.

RAVINDRA V. GINDE, M.D. (Authorization No. 116876-4W)

Please be advised that, effective June 28, 2007, Dr. Ravindra Ginde permanently surrendered his license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine, pursuant to New York State Public Health Law Section 230.13.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Ginde from the list of authorized physicians, effective October 2, 2007. Dr. Ginde's current address is 11 Holmes Street, West Orange, New Jersey 07052.

Dr. Ginde is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Ginde for treatment or examinations rendered prior to June 28, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Ginde regarding treatment or examinations he rendered prior to June 28, 2007, should not be denied due to the surrender of his license or his removal from the list of authorized physicians.

BRAD J. JACOBS, M.D. (Authorization No. 175524-8W)

Please be advised that, by Surrender Order of the New York State Department of Health, State Board for Professional Medical Conduct, effective September 21, 2007, Dr. Brad Jacobs surrendered his license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Jacobs from the list of authorized physicians, effective September 28, 2007. VIDYADHARA A. KAGALI, M.D. (Authorization No. 122498-9W)

Please be advised that, by Modification Order (BPMC No. 04-185), effective September 11, 2007, the New York State Department of Health, State Board for Professional Medical Conduct, modified the terms of Order No. 04-185 previously imposed on August 27, 2004. This Modification Order does not constitute a new disciplinary action. Dr. Vidyadhara Kagali's license to practice medicine in the State of New York is suspended until the successful completion of a residency program, after which the suspension will be lifted and Dr. Kagali will be placed on probation for two years.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Kagali from the list of authorized physicians, effective October 8, 2007. Dr. Kagali's current address is 159 Altamont Drive, Hurley, New York 12443.

Dr. Kagali is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kagali for treatment or examinations rendered prior to September 11, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Kagali regarding treatment or examinations he rendered prior to September 11, 2007, should not be denied due to the suspension of his license or his removal from the list of authorized physicians.

VENKATESWARA KOLLI, M.D. (aka Venkat R. Kolli) (Authorization No. 198308-9W)

Please be advised that, by Consent Order, adopting Consent Order and Agreement (BPMC No. 07-195), of the New York State Department of Health, State Board for Professional Medical Conduct, effective September 11, 2007, Dr. Venkateswara Kolli did not contest the charge of negligence on more than one occasion. The penalty imposed is probation for five years.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Kolli from the list of authorized physicians, effective September 28, 2007. Dr. Kolli's current address is 620 Tenth Street, Suite 700, Niagara Falls, New York 14301.

Dr. Kolli is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Kolli for treatment or examinations rendered prior to September 28, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Kolli regarding treatment or examinations he rendered prior to September 28, 2007, should not be denied due to his removal from the list of authorized physicians.

GLENN SCHROYER, M.D. (Authorization No. 187467-6W)

Please be advised that, by Consent Order (BPMC No. 07-167) of the New York State Department of Health, State Board for Professional Medical Conduct, effective August 15, 2007, Dr. Glenn Schroyer's license to practice medicine in the State of New York was suspended for thirty-six months with the last thirty-two months stayed with probation for thirty-six months. Dr. Schroyer admitted guilt to the charges of gross negligence; failure to exercise appropriate supervision; and failure to maintain accurate records.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Schroyer from the list of authorized physicians, effective September 28, 2007. Dr. Schroyer's current address is 20 Glen Drive, Plattsburgh, New York 12901.

Dr. Schroyer is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Schroyer for treatment or examinations rendered prior to August 15, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Schroyer regarding treatment or examinations he rendered prior to August 15, 2007, should not be denied due to the suspension of his license or his removal from the list of authorized physicians.

M. JOSEPH VITOULIS, D.O. (Authorization No. 205528-3B)

Please be advised that, by Determination and Order (BPMC No. 07-196), a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct, sustained charges finding Dr. M. Joseph Vitoulis guilty of practicing fraudulently; performing unwarranted tests and treatment; and failing to maintain accurate patient records. Effective September 17, 2007, Dr. Vitoulis's license to practice medicine in the State of New York was suspended for three years, with the last two and one half years stayed with probation for three years and a $50,000 fine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Vitoulis from the list of authorized physicians, effective September 28, 2007. Dr. Vitoulis's current address is 1030 Franklin Avenue, Apartment 3, Valley Stream, New York 11580.

Dr. Vitoulis is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Vitoulis for treatment or examinations rendered prior to September 17, 2007, are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Vitoulis regarding treatment or examinations he rendered prior to September 17, 2007, should not be denied due to the suspension of his license or his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through September 16, 2007, may be provided.
