MICHAEL E. CONRAD, M.D. (Authorization No. 164820-3W)

Please be advised that, by Surrender Order (BPMC No. 07-89) of the New York State Department of Health, State Board for Professional Medical Conduct, effective May 2, 2007, Dr. Michael Conrad surrendered his license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Conrad from the list of authorized physicians, effective May 8, 2007. Dr. Conrad's current address is 8012 Third Avenue, Brooklyn, New York 11209.

Dr. Conrad is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Conrad for treatment or examinations rendered prior to May 2, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Conrad regarding treatment or examinations he rendered prior to May 2, 2007 should not be denied due to the surrender of his license or his removal from the list of authorized physicians.

HENRY J. DOBIES, M.D. (Authorization No. 076017-3W)

Please be advised that, by Determination and Order (BPMC No. 07-94), a Hearing Committee of the New York State Department of Health, State Board for Professional Medical Conduct, revoked Dr. Henry Dobies' license to practice medicine in the State of New York, or in any other jurisdiction where that practice is predicated on a valid New York State license to practice medicine, effective May 8, 2007.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Dobies from the list of authorized physicians, effective May 8, 2007. Dr. Dobies' current address is 173 East Orvis Street, Massena, New York 13662.

Dr. Dobies is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Dobies for treatment or examinations rendered prior to May 8, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Dobies regarding treatment or examinations he rendered prior to May 8, 2007 should not be denied due to the revocation of his license or his removal from the list of authorized physicians.

GEORGE S. PARLATO, M.D. (Authorization No. 087101-2B)

Please be advised that, by Consent Order (BPMC No. 07-72) of the New York State Department of Health, State Board for Professional Medical Conduct, effective April 16, 2007, George S. Parlato, M.D.'s license was limited to preclude him from the practice of medicine, clinical or otherwise, in the State of New York.

Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to remove Dr. Parlato from the list of authorized physicians, effective May 10, 2007. Dr. Parlato's current address is 13418 Quaker Street, Collins, New York 14034.

Dr. Parlato is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Parlato for treatment or examinations rendered prior to April 16, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Parlato regarding treatment or examinations he rendered prior to April 16, 2007 should not be denied due to the limitation of his license or his removal from the list of authorized physicians. In addition, testimony and/or depositions concerning independent medical examinations conducted through April 15, 2007 may be provided.
