VOLUNTARY RESIGNATION — VLADIMIR MARGOLIN, M.D.

Please be advised that, effective from October 2, 2006 through October 1, 2007, Dr. Margolin's authorization (Number 199935-8W) to treat workers' compensation claimants is suspended by the New York State Workers' Compensation Board pursuant to Workers' Compensation Law Section 13-d(4).

Effective March 29, 2007, Dr. Margolin voluntarily resigned his authorization to treat workers' compensation claimants. His current address is 10 Greenway, Oyster Bay, New York 11771-4604.

Dr. Margolin is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Margolin for treatment or examinations rendered prior to October 2, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Margolin regarding treatment or examinations he rendered prior to October 2, 2006 should not be denied due to the suspension of his authorization or this resignation.

TEMPORARY SUSPENSION — DANIEL CHENG, M.D.

Please be advised that, effective from November 11, 2006 through February 8, 2007, by Consent Order (BPMC No. 06-255) of the New York State Department of Health, State Board for Professional Medical Conduct, Dr. Cheng's license to practice medicine in the State of New York was suspended.

Effective April 19, 2007 through April 18, 2008, Dr. Cheng's authorization (Number 126436-5W) to treat workers' compensation claimants is suspended by the New York State Workers' Compensation Board pursuant to Workers' Compensation Law Section 13-d(4). Dr. Cheng's current address is 110 Third Street, Waterford, New York 12188.

Dr. Cheng is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Cheng for treatment or examinations rendered prior to April 19, 2007 are valid, except during the actual license suspension period of November 11, 2006 through February 8, 2007, but are otherwise invalid for any treatment or examinations rendered on or after April 19, 2007. Requests to cross-examine Dr. Cheng regarding treatment or examinations he rendered prior to April 19, 2007, except for treatment or examinations between November 11, 2006 and February 8, 2007, should not be denied due to the suspension of his medical license or this suspension.

TEMPORARY SUSPENSION — THOMAS V. ROBB, D.O.

Please be advised that, by Determination and Order (BPMC No. 06-176), the New York State Department of Health, State Board for Professional Medical Conduct, suspended Dr. Robb's license to practice in the State of New York for three (3) months, effective from August 1, 2006 through October 31, 2006.

Effective April 16, 2007, through April 15, 2008, Dr. Robb's authorization (Number 209975-2B) to treat and conduct independent medical examinations of workers' compensation claimants is suspended by the New York State Workers' Compensation Board pursuant to Workers' Compensation Law Section 13-d(4). Dr. Robb's current address is 1441 Lattintown Road, Newburgh, New York 12550.

Dr. Robb is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Robb for treatment or examinations rendered prior to August 1, 2006 are valid, except during the actual license suspension period of August 1, 2006 though October 31, 2006, but are otherwise invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Robb regarding treatment or examinations he rendered prior to April 16, 2007, except for treatment and examinations from August 1, 2006 through October 31, 2006, should not be denied due to the suspension of his medical license or this suspension. In addition, testimony and/or depositions concerning independent medical examinations conducted through July 31, 2006 may be provided.
