VOLUNTARY RESIGNATION - DOCTOR FREDERICK DOWNS, M.D.

Please be advised that, effective April 16, 2007, Frederick Downs, M.D., voluntarily resigned his authorization (Number 109864-9W) to treat workers' compensation claimants. Dr. Downs's current address is 20431 Saltsdale Road, Umatilla, Florida 32784.

Dr. Downs is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Downs for treatment or examinations rendered prior to April 16, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Downs regarding treatment or examinations he rendered prior to April 16, 2007 should not be denied due to this resignation.

VOLUNTARY RESIGNATION - DOCTOR JEFFREY S. SCHWARTZ, M.D.

Please be advised that, effective May 31, 2006, the New York State Department of Health, State Board for Professional Medical Conduct (BPMC No. 06-068), revoked Jeffrey S. Schwartz, M.D.'s, license to practice medicine in the State of New York.

Effective April 17, 2007, Dr. Schwartz voluntarily resigned his authorization (Number 152188-9W) to treat workers' compensation claimants. His current address is 1233 Beech Street #42, Atlantic Beach, New York 11509.

Dr. Schwartz is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Schwartz for treatment or examinations rendered prior to May 31, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Schwartz regarding treatment or examinations he rendered prior to May 31, 2006 should not be denied due to the revocation of his license or this resignation.

VOLUNTARY RESIGNATION – DOCTOR SALVATORE A. FEUDI, D.C.

Please be advised that, by Order (No. 23011), effective December 5, 2006, the New York State Board of Regents accepted the surrender of Salvatore A. Feudi, D.C.'s license to practice chiropractic in the State of New York.

Effective April 23, 2007, Dr. Feudi voluntarily resigned his authorization (No. C06391-9W) to treat workers' compensation claimants. His current address is Bare Hill Correctional Facility, DIN #03A5653, Caller Box 20, 181 Brand Road, Malone, New York 12953.

Dr. Feudi is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Feudi for treatment or examinations rendered prior to December 5, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Feudi regarding treatment or examinations he rendered prior to December 5, 2006 should not be denied due to the surrender of his license or this resignation.
