Voluntary Resignation of Doctors' Authorization
to Treat Workers' Compensation Claimants

VOLUNTARY RESIGNATION — ANNE BRUTUS, M.D

Please be advised that, effective October 6, 2006, Dr. Anne Brutus permanently surrendered her license to practice medicine in New York State or in any other jurisdiction where the practice of medicine is predicated on a valid New York State medical license to the State Board for Professional Medical Conduct.

Effective February 22, 2007, Dr. Brutus voluntarily resigned her authorization (Number 133269-1W) to treat workers' compensation claimants. Dr. Brutus's current address is 450 McDermott Road, Rockville Centre, New York 11570-1426.

Dr. Brutus is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Brutus for treatment or examinations rendered prior to October 6, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Brutus regarding treatment or examinations she rendered prior to October 6, 2006 should not be denied due to the surrender of her license or this resignation.

VOLUNTARY RESIGNATION — NICHOLAS FILANNINO, M.D.

Please be advised that, effective February 27, 2007, the State Board for Professional Medical Conduct issued a Nondisciplinary Order of Conditions wherein Dr. Filannino agreed to refrain from any and all medical practice, clinical or otherwise, in the State of New York. Effective March 2, 2007, Dr. Filannino voluntarily resigned his authorization (Number 089298-4W) to treat workers' compensation claimants. Dr. Filannino's current address is 18 Bunker Hill Circle, Shelton, Connecticut 06484.

Dr. Filannino is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Filannino for treatment rendered prior to February 27, 2007 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Filannino regarding treatment he rendered prior February 27, 2007 should not be denied due to the above Order or this resignation.

VOLUNTARY RESIGNATION — S. RAMACHANDRAN NAIR, M.D.

Please be advised that, effective March 2, 2007, Dr. Nair voluntarily resigned his authorization (Number 131614-0W) to treat workers' compensation claimants. Dr. Nair's office is located at 11 Ralph Place, Suite 211, Staten Island, New York 10304.

Dr. Nair is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Nair for treatment or examinations rendered prior to March 2, 2007 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Nair regarding treatment or examinations he rendered prior to March 2, 2007 should not be denied due to this resignation.

VOLUNTARY RESIGNATION — JOHN K. VARGHESE, M.D.

Please be advised that, effective March 22, 2006, by Consent Order (BPMC No. 06-58), the State Board for Professional Medical Conduct issued a censure and reprimand to Dr. John Varghese with a permanent license limitation to practice only anesthesiology and an order to close his practice no later than April 19, 2006. Effective March 7, 2007, Dr. Varghese voluntarily resigned his authorization (Number 149674-4W) to treat workers' compensation claimants. Dr. Varghese's current address is 20 Hancock Drive, Horseheads, New York 14845.

Dr. Varghese is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Varghese for treatment rendered prior to April 19, 2006 are valid, but are invalid for any treatment rendered on or after that date. Requests to cross-examine Dr. Varghese regarding treatment he rendered prior to April 19, 2006 should not be denied due to the above Order or this resignation.
