VOLUNTARY RESIGNATION – ARNOLD B. CAMPO, M.D.

Please be advised that, effective November 28, 2006, pursuant to a Modification Order of the State Board for Professional Medical Conduct (BPMC No. 03-133), Dr. Arnold Campo agreed never to register or reapply for a license to practice medicine in New York State. His license became inactive as of that date.

Effective February 2, 2007, Dr. Campo voluntarily resigned his authorization (Number 206040-8W) to treat workers' compensation claimants. His current address is 1718 Partridge Hill, Lancaster, South Carolina 29720.

Dr. Campo is prohibited from rendering medical or other treatment and care to individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Campo for treatment or examinations rendered prior to November 28, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Campo regarding treatment or examinations he rendered prior to November 28, 2006 should not be affected by the inactive status of his medical license or this resignation.

VOLUNTARY RESIGNATION – SHOLOM GOOTZEIT, D.O.

Please be advised that, effective December 25, 2006, the State Board for Professional Medical Conduct issued a Consent Order (BPMC No. 06-290) wherein Dr. Sholom Gootzeit's license to practice medicine was limited to preclude him from treating or billing patients in New York State whose services are reimbursed through no-fault insurance and/or workers' compensation.

Effective February 3, 2007, Dr. Gootzeit voluntarily resigned his authorization (Number 193515-4B) to treat and conduct independent medical examinations of workers' compensation claimants. His current address is 19475 North Grayhawk Drive, Unit 1016N, Scottsdale, Arizona 85255-7415.

Dr. Gootzeit is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work-related injuries or illnesses. Medical reports submitted by Dr. Gootzeit for treatment or examinations rendered prior to December 25, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Gootzeit regarding treatment or examinations he rendered prior to December 25, 2006 should not be denied due to the limitation of his license or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through December 24, 2006 may be provided.
