VOLUNTARY RESIGNATION – DR. HARVEY GRABLE

Please be advised that, effective January 18, 2007, Harvey Grable, M.D. voluntarily resigned his authorization (Number 095120-2B) to treat and conduct independent medical examinations of workers' compensation claimants. Dr. Grable's license to practice medicine in New York State was previously deemed inactive, effective November 15, 2006, by Modification of Order (BPMC No. 00-91). His current address is 6985 SE Lakeview Terrace, Stuart, Florida 34996.

Dr. Grable is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Grable for treatment or examinations rendered prior to November 15, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Requests to cross-examine Dr. Grable regarding treatment or examinations he rendered prior to the effective date of his license inactivity should not be denied due to the Modification of Order or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through November 14, 2006 may be provided.

VOLUNTARY RESIGNATION – DR. HARVEY LEVINE

Please be advised that, effective November 28, 2006, the State Board for Professional Medical Conduct issued a Consent Order (BPMC No. 06-262) wherein Harvey Levine, M.D.'s license to practice medicine was, in part, limited to preclude him from treating patients in New York State. The Order did, however, allow Dr. Levine to conduct independent medical examinations on behalf of third-party administrators engaged by insurance companies to issue reports documenting the results of such examinations.

Effective January 25, 2007, Dr. Levine voluntarily resigned his authorization (Number 089213-3B) to treat and conduct independent medical examinations of workers' compensation claimants. His current residence is located at One Columbus Place, Apt. 543C, New York, New York 10019.

Dr. Levine is prohibited from rendering medical or other treatment and care to and conducting independent medical examinations of individuals who have suffered work–related injuries or illnesses. Medical reports submitted by Dr. Levine for treatment rendered prior to November 28, 2006 are valid, but are invalid for any treatment or examinations rendered on or after that date. Medical reports submitted by Dr. Levine for independent medical examinations rendered prior to January 25, 2007 are valid, but are invalid for any independent medical examinations rendered on or after that date. Requests to cross-examine Dr. Levine regarding treatment or examinations he rendered prior to November 28, 2006 should not be denied due to the limitation of his license or this resignation. In addition, testimony and/or depositions concerning independent medical examinations conducted through January 24, 2007 may be provided.
