Please be advised that physical therapist (PT) Chris Capilli has resigned his authorization to treat injured workers in the New York State workers' compensation system effective December 5, 2022. PT Capilli's authorization to treat injured workers was previously temporarily suspended effective August 9, 2022. See Subject Number 046-1535. PT Capilli's last known practice address is:
