Revocation of Dr. Muneer Imam

Authorization No. 159557- 8W

August 7, 2018

Pursuant to Workers' Compensation Law Section 13-d (5), the Chair of the New York State Workers' Compensation Board has exercised her authority to revoke Dr. Muneer Imam’s authorization to treat injured workers within the New York State workers’ compensation system effective August 6, 2018, on the grounds that the New York State Department of Health issued a determination that included findings of professional misconduct by Dr. Imam.

Dr. Imam's last known business addresses are:

2 Union Avenue, Ctr. Moriches, New York 11934, and
131 Sunnyside Boulevard, Suite 100, Plainview, New York 11803

The revocation is based upon a Determination and Order (BPMC No. 18-164), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective July 26, 2018, wherein the Hearing Committee sustained the first through ninth and 11th through 19th specifications of misconduct, finding that Dr. Imam violated the terms of his probation by practicing medicine without an approved Practice Monitor, and failed to maintain required insurance coverage. This resulted in a six-month suspension, an $18,000.00 fine, and a three-year probationary period, during which Dr. Imam may practice medicine only when monitored by a licensed physician, board certified in an appropriate specialty (“Practice Monitor”), and during which he shall maintain medical malpractice insurance coverage as ordered.

Dr. Imam is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Imam for services rendered prior to August 6, 2018 are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Imam for services rendered prior to August 6, 2018 should not be denied due to the revocation of his authorization.

Any questions regarding the authorized lists should be referred to the Medical Director’s Office at 1 (800) 781-2362.
