Pursuant to Workers' Compensation Law Section 13-d(5), the Chair of the New York State Workers' Compensation Board has exercised her authority to revoke Dr. James Vevaina’s authorization to treat injured workers within the New York State workers’ compensation system effective December 22, 2017. Dr. Vevaina's last known business address is:

The revocation is based upon Consent Order (BPMC No. 17-338), issued by the New York State Department of Health, State Board for Professional Medical Conduct (BPMC), effective December 12, 2017, wherein Dr. Vevaina must surrender his license to practice medicine, based on a California Board Order, pursuant to which it was concluded that he committed acts of gross negligence, repeated acts of negligence, and failed to maintain adequate and accurate records regarding his care of three patients.

Dr. Vevaina is prohibited from rendering treatment and care to injured workers. Reports submitted by Dr. Vevaina for services rendered prior to December 22, 2017 are valid, but are invalid for any services rendered on or after that date. Requests to cross-examine Dr. Vevaina for services rendered prior to December 22, 2017 should not be denied due to the revocation of his authorization.
