Web Submission Process Tutorial
Accessing the form
Select "Submit EC-4NARR" from the "Login for Access To: " selection box on the Board's home page;
or
On the Home Page, select
then select "Doctor's Narrative Report" under On-Line Services
select Web Submission
and at the bottom of the next page select
Enter user ID and Password to login.
The form will open in a web browser.
